Comparative Study of Treatment Results between Surgical Neck Clipping and Endovascular Coiling for Intracranial Aneurysms -Early experience during the learning curve-

Abstract
This study is designed to compare the treatment results between surgical neck clipping and endovascular coiling for intracranial aneurysms during the learning curve. Patients who admitted due to intracranial aneurysm between January 2006 and August 2007 were selected. They have done surgical neck clipping or endovascular coiling by the author. A retrospective study including the inpatient and outpatient medical record, radiologic examination was performed. Clinical state was determined by Hunt-Hess grade (H-H) and Glasgow outcome scale (GOS). Twenty-four cases (twenty-three patients) were included in this study. The average age was 59.08 years and there were 8 men and 15 women. The most common location of aneurysm was anterior communicating artery (8) and middle cerebral artery (5). Surgical neck clipping was done for 9 cases and endovascular coiling was done for 15 cases. Ruptured aneurysms were 14 cases, and unruptured aneurysms were 10 cases. There was no specific difference in the treatment results and complication between two groups. But, duration of the procedure and hospitalization were longer in the surgical treatment group.
